The Merrimack College Athletic Training Department has recently moved to an online injury-tracking system, Sportsware. This system will benefit our student-athletes, coaches, and staff in many ways, particularly with yearly documentation.

If you have already submitted paper documentation you will still need to create your online identification and complete the following steps:

Prior to participating on a team for Merrimack College, all student athletes must provide the Athletic Training Department with the following information found within the website:

  • General Information
  • Current Address
  • Emergency Contact 
  • Insurance
  • Medical Alert
  • Health History Information
  • Electronically sign a consent to treat form under the “forms” section of the website

In addition, within the website the following forms need to be UPLOADED by you:

Incoming Freshmen/Transfers/Walk-Ons

  • Physical (Within 6 months of first team activity)
  • Sickle Cell Form - Testing or Waiver
  • Copy of insurance card(s) (front and back!)
  • ADHD letter (if necessary)

Returners

  • Copy of insurance card(s) (front and back!)
  • ADHD letter (if necessary)

Fifth-Year Student-Athletes

  • Physical (Within 6 months of first team activity)
  • Copy of insurance card(s) (front and back!)
  • ADHD letter (if necessary)

This documentation MUST be completed by Friday, August 1st 2014.

MEDICAL FORMS

Physical Exam: To be completed by all freshmen, transfers, walk-ons and fifth-year student-athletes. This form is to be completed by your Primary Care Physician WITHIN SIX MONTHS from your reporting date to campus. You must bring this form with you to your primary care physician.

Sickle Cell Forms: Please review and complete the following forms regarding Sickle Cell:

ADHD Policy: Please review if you have been diagnosed with ADHD and bring to your diagnosing physician to obtain appropriate documentation.

ABOUT THE SPORTS MEDICINE DEPARTMENT

The newly-reaccredited division of Sports Medicine and Athletic Training at Merrimack College is designed with a dual purpose. The department is both a fully-functioning athletic training room that provides health care services for approximately 600 Merrimack College student-athletes participating in 24 varsity intercollegiate sports and an educational facility which provides a top-notch athletic training curriculum with an emphasis on allied health education.

Student-athletes and Merrimack receive the finest medical care available for the prevention, diagnosis, and treatment of athletics injuries. Director of Sports Medicine Birgid Hopkins, Clinical Coordinator of Health Science Dennis Fontaine and Head Athletic Trainer Allan Del Valle supervise the sports medicine staff which consists of five BOC Certified Athletic Trainers and approximately 30 athletic training students. The staff is committed to ensuring quality care for student-athletes while enabling a safe and healthy participating in intercollegiate athletics.

ABOUT THE MERRIMACK SPORTS MEDICINE MAJOR

Merrimack’s sports medicine program is in its 15th year of offering a field of study that will allow the athletic training student a wide variety of academic and clinical programs that will provide knowledge and skills in accordance with National Athletic Trainers Association competencies and Commission on Accreditation of Athletic Training Education Programs guidelines.

Some of Merrimack’s alumni have gone on to assume positions at University of Tennessee, Florida State University, University of Maryland, Palmer School of Chiropractic, and many other Allied Heath Care programs, including physical therapy, physician assistant, and nurse practioner.
